Join DataShare Santa Cruz County and CORE Investments in a community conversation about harnessing local data to foster Health & Wellness (one of the eight CORE Conditions for Health & Well-being) so that all community members in Santa Cruz County experience optimal physical, mental, social-emotional, behavioral, and spiritual health across the life span.
We will explore local, regional, state and national data in the CORE Results Menu on DataShare in order to gather insights, identify trends, and make meaning of data in these Health & Wellness impact areas:
Equitable access to affordable, quality care (e.g., insurance coverage, medical “home”)
Appropriate utilization of care (e.g., early prenatal care, screenings, oral health)
Behaviors that maintain or improve health (e.g., breastfeeding, immunizations, healthy eating, active living, diabetes monitoring, decreased substance use)
Optimal health status (e.g., birth outcomes, teen birth rates, physical and behavioral health status, disabilities, mortality rates)
In this interactive session, participants will be asked to share their perspectives on these questions:
What “stories” do the selected indicators tell about community strengths, needs, and root causes of differences in well-being based on race, ethnicity, age, gender, sexual orientation, geographic area, or other “equity dimensions?”
What programs, practices, and policies exist (or are needed) to create the conditions for optimal Health & Wellness for all community members across the lifespan?
How could we build on the available data to measure change, learn what works, and assess our collective impact?
This will be the first in a 6-part community conversation series to dive into the data that can help people identify community strengths and needs, set goals for community well-being, track progress, and connect strategies across the CORE Conditions for Health and Well-being.
